Nelangie Cares LLC ·

Figures as of September 20, 2026. Draft and proposed items are marked as such.

The 2027 Star Ratings cut points shown are draft (plan preview); final ratings were expected in early October 2026. MSSP APP Plus items come from the CY 2027 PFS proposed rule (final expected about Nov 1, 2026). Educational only; not legal, actuarial, or coding advice.

About this guide

Improving is no longer enough. For the draft 2027 Star Ratings, half of all cut points moved up, and a plan or practice that performs exactly as well as last year can lose a half star anyway. Because Stars is graded on a curve, the question has shifted from "how do we improve our Stars" to "how do we improve faster than the market."

This guide covers the CMS and NCQA quality changes that decide Medicare Advantage revenue in 2027: the 2027 and 2028 Star Ratings measure sets and weights, the cut-point movement, the removal of eleven measures, the cancelled Health Equity Index reward, risk-adjusted adherence measures, and the HEDIS MY2026 and MY2027 specifications. It is organized as three sets of questions, for primary care providers, ACO executives, and payors, and every answer ends with how Nelangie Cares closes the gap. Final rules are marked final; draft and proposed items are marked as such.

CY 2027 Stars, HEDIS and quality at a glance

The bar rose for the second year running, the measures that remain are clinical, and the reward for equity work is gone. The numbers behind every answer below:

What the six areas in the reference infographic look like in practice: provider performance, member engagement, quality operations, data and analytics, care management, and closing gaps in care are not six programs. They are one workflow inside the practice, and that is where every answer below lands.

Questions from primary care providers

For a primary care practice, Stars and HEDIS are the terms of your quality incentive, your capitation withhold, and increasingly your network status. Every answer below ends with how Nelangie Cares closes the gap.

Q1My quality scores went up last year. Why did my plan tell me my incentive is at risk?

Because the bar moved more than you did. For the draft 2027 Star Ratings, half of all cut points rose, almost entirely on HEDIS measures: Kidney Health Evaluation moved 7 to 10 points, Colorectal Cancer Screening 4 to 11 points, Controlling Blood Pressure 4 to 5 points. A practice that held a 72% blood pressure control rate may now sit below the threshold the plan needs from its network. Plan incentive contracts are written against those thresholds, so the same performance pays less.

How Nelangie Cares closes the gap

We reset your targets against the new cut points, not last year's, and convert them into a monthly working list by measure: who is uncontrolled, who is due, who has no kidney evaluation on file. We redesign the rooming workflow so a blood pressure recheck, a urine albumin order, and a colorectal screening order happen before the provider walks in. The goal is a practice that improves faster than the curve, which is the only way to keep an incentive that is graded relative to everyone else.

MeasuresKED: Kidney Health EvaluationCOL-E: Colorectal Cancer ScreeningCBP: Controlling High Blood Pressure
Q2Which measures should my practice own in 2027, and which ones stopped mattering?

What stopped mattering are administrative measures you never controlled: call center, appeals, complaints, disenrollment, and two CAHPS items. What is left is clinical and lands in your exam room: Controlling Blood Pressure, Diabetes glycemic control, Kidney Health Evaluation, Eye Exam (chart review no longer counts), breast and colorectal screening, three medication adherence measures, and the new Functional Status Assessment, Polypharmacy, and Opioid-Benzodiazepine measures. The two HOS survey measures, Improving or Maintaining Physical Health and Mental Health, now weigh 3x, and Depression Screening and Follow-Up arrives for the 2029 ratings.

How Nelangie Cares closes the gap

We build your measure set into your EHR templates so the data lives in structured fields, not free text: functional status and fall risk at every Medicare visit, PHQ screening with documented follow-up, a deprescribing review for anticholinergics and opioid-benzodiazepine overlap, and a diabetic eye exam result captured from the ophthalmologist rather than a chart note. Because eye exam chart review is gone and NCQA is retiring chart-sample reporting for blood pressure by MY2028, we make structured capture the default now.

MeasuresCBP: Controlling High Blood PressureGSD: Glycemic Status AssessmentKED: Kidney Health EvaluationEED: Eye Exam for Patients with DiabetesBCS-E: Breast Cancer ScreeningCOL-E: Colorectal Cancer ScreeningADH-3: Medication Adherence (diabetes / RASA / statins)COA: Care for Older AdultsDSF-E: Depression Screening & Follow-Up
Q3The adherence measures are becoming risk-adjusted. Does that help or hurt my incentive?

For measurement year 2026, the three Part D adherence measures (diabetes medications, RAS antagonists, statins) add sociodemographic risk adjustment and drop the inpatient and SNF stay adjustments. A practice with a low-income or dual-eligible panel will be judged against an adjusted expectation, which helps. But the adjustment removes the excuse: a plan can no longer accept poor adherence in your panel as a patient-mix problem, and it will bring adherence targets into your contract. These measures return to 3x weight in the 2029 ratings.

How Nelangie Cares closes the gap

We fold adherence into the care management program: medication reconciliation and refill coordination as standard chronic care management tasks, 90-day fills and pharmacy synchronization requested at the visit, and outreach in the patient's language with AI-supported interpretation and nurse oversight. We coordinate the calendar with the plan's pharmacy team so members are not called twice and gaps are closed before the measurement window ends.

MeasuresADH-3: Medication Adherence (diabetes / RASA / statins)
Q4My patients speak Russian, Spanish, and Creole. How do I move survey-based measures I cannot see?

The HOS physical and mental health measures now weigh 3x, are fielded by mail and phone only, and compare a patient's own answers two years apart. CAHPS is web, mail, then phone. Neither survey has a workflow you can chart, but both are driven by things you do control: whether the patient feels their chronic conditions are managed, whether they got timely appointments and clear answers, whether someone followed up after a hospital stay, and whether they could communicate in their language.

How Nelangie Cares closes the gap

We treat the survey measures as care-experience measures and build the drivers into the visit: functional status and falls addressed with a plan, depression screened and followed up, same-week access for chronic patients, and after-visit summaries in the patient's language. Our multilingual outreach model was built for panels like yours, and the same care management touches that close clinical gaps are what patients remember when the survey arrives.

Q5How much of my revenue actually rides on this, and what does a half-star cost me?

For the plan, a half-star drop below 4 stars removes the 5% quality bonus and reduces the rebate share from 65% to 50% of the bid-benchmark difference; the average bonus in 2026 is $381 per enrollee in individual plans, and only 68% of enrollees are in bonus-status plans, down from 75%. Plans pass that through as withholds, tighter incentive tiers, and benefit cuts that push members to switch. For a capitated practice on a percent-of-premium deal, the plan's lost bonus is your lost premium base.

How Nelangie Cares closes the gap

We start with a revenue exposure review that maps each of your contracts to the measures and thresholds it pays on, quantifies what a half-star swing does to your incentive and capitation, and prioritizes the measures with the largest dollar swing per patient. We then run the practice-level program that moves those measures month by month, with reporting your plan partners can see, so your practice is the one they protect when the network narrows.

Q6NCQA is going digital. What does that mean for how my staff documents?

NCQA is retiring chart-sample (hybrid) reporting: Statin Therapy measures went ECDS-only in MY2026, Controlling Blood Pressure and Blood Pressure Control for Diabetes move to ECDS in MY2028, Glycemic Status, Transitions of Care, and Care for Older Adults in MY2029, with fully digital HEDIS by MY2030. In plain terms, a blood pressure written in a progress note will stop counting; only a structured, coded value transmitted electronically will. Social need screening moves from the G0136 code to LOINC-coded results in MY2027.

How Nelangie Cares closes the gap

We audit where your quality data actually lives in the EHR, fix the templates so vitals, screenings, and results are captured in discrete fields with the right codes, and confirm the data flows to your plan and ACO partners through supplemental data feeds. We train the team in a two-day onboarding and then check the structured capture rate monthly so the practice is ready before each hybrid measure retires, not after.

Questions from ACO executives

An ACO carries quality on two ledgers at once: the Medicare program it reports to (LEAD from January 1, 2027, or MSSP) and the MA value-based contracts whose withholds and gates are written against Star measures. Both ledgers moved for 2027, and the same practice-level data now feeds both.

Q1ACO REACH ends December 31, 2026. How is quality scored in LEAD, and what is at stake?

LEAD (Long-term Enhanced ACO Design) runs PY2027 through PY2036 with a 3% quality withhold on the benchmark for every ACO, earned back on performance; unearned withhold funds a High Performers Pool. The measure set keeps five REACH measures (risk-standardized all-condition readmissions, unplanned admissions for patients with multiple chronic conditions, days at home for patients with complex chronic conditions, timely follow-up after acute exacerbations, and CAHPS) and adds two MSSP eCQMs: Controlling High Blood Pressure and Diabetes glycemic status above 9%. The eCQMs are optional in 2027 and 2028, pay-for-reporting in 2029 and 2030, and pay-for-performance from 2031 through 2036. The score is up to 10 points per measure, adjusted by a continuous improvement and sustained exceptional performance multiplier, plus an eCQM reporting adjustment (up to 2.5 points in the first two years) and a Prevention and Quality Plan adjustment (up to 5 points in 2027 and 2028, up to 10 afterward). HOS is not in the set; CAHPS is. The REACH health equity benchmark adjustment and health equity plan do not carry over.

How Nelangie Cares closes the gap

We build the two eCQMs into your practices now, while they are optional, so structured blood pressure and A1c data are flowing and validated long before pay-for-performance in 2031, and we capture the eCQM reporting adjustment from year one. For the claims-based measures we run transitional care within 48 hours of discharge, follow-up within 7 to 14 days of an exacerbation, and chronic care management for the multi-condition population, and we report each practice's contribution to readmissions and unplanned admissions monthly.

MeasuresCBP: Controlling High Blood PressureGSD: Glycemic Status AssessmentTFU: Timely Follow-Up after Acute Exacerbation
Q2What is the LEAD Prevention and Quality Plan, and how do we earn the points?

Every LEAD ACO must adopt a Prevention and Quality Plan on one of three tracks: cardiovascular disease, falls, or nutrition. The plan is worth up to 5 quality points in 2027 and 2028 and up to 10 from 2029, which is a meaningful share of a score built on 10 points per measure. Benefit enhancements support each track: medical nutrition therapy for prediabetes and hyperlipidemia (new for Global ACOs in 2027), the chronic disease prevention reward (healthy food up to $150), and the RISE to Age in Place falls-prevention episode under CARA from 2028 (up to $2,500 in home interventions over three to six months).

How Nelangie Cares closes the gap

We choose the track that matches your population and your existing Star measure work, then build it into rooming steps rather than a document filed with CMS. The cardiovascular track maps directly to our Controlling Blood Pressure and statin adherence workflows; the falls track maps to the functional status and fall risk screening we already run for the new Star Functional Status Assessment measure and the 3x-weighted HOS measures. One workflow earns LEAD quality points and Star credit at the same time.

MeasuresCBP: Controlling High Blood PressureADH-3: Medication Adherence (diabetes / RASA / statins)COA: Care for Older Adults
Q3Where do we stand in the LEAD timeline, and what should we do in the implementation period?

Applications closed May 17, 2026; CMS said it would notify selected ACOs by early summer and, as of September 2026, has not published a participant list. The deadline to resolve TIN overlap between MSSP and LEAD rosters was September 8, 2026 (a TIN cannot sit in both). The optional implementation period runs September 15 to December 31, 2026 with no alignment, payment, or risk, and PY2027 begins January 1, 2027. CARA target-price reports arrive in Q2 2027 and the first CARA performance year is 2028. CMS anticipates future application windows but has not announced dates.

How Nelangie Cares closes the gap

If you were accepted, we use the implementation period for participant TIN readiness reviews, eCQM data capture audits, and the care management build a 3% withhold and first-dollar risk demand. If you were not selected, we build the MSSP transition plan, including the one-time BASIC/ENHANCED switch CMS proposed for January 2027 applicants, and a quality readiness roadmap for a future LEAD cohort.

Q4We are staying in MSSP. What changes in APP Plus for 2027, and are we ready?

Under the CY 2027 PFS proposed rule, APP Plus shrinks to eight measures for PY2027: five clinical measures reportable as eCQM, MIPS CQM, Medicare CQM, or the new Medicare eCQM option, CAHPS for MIPS (now web, mail, and phone), and two administrative claims measures; Quality ID 305 (SUD treatment) and 493 (Adult Immunization Status) are removed. MIPS CQMs are extended with the reporting incentive, all Medicare CQMs move to flat benchmarks, and CEHRT can be met through three pathways. The 40th-percentile performance threshold still gates shared savings. FHIR-based reporting is expected to be mandatory around 2030, so the data plumbing you build now has to last.

How Nelangie Cares closes the gap

We run an eCQM readiness audit across participant EHRs, find where blood pressures, A1c results, and screenings are typed as free text or stored in the wrong field, and fix templates and workflows at the source. We stand up a practice-level APP Plus dashboard tracked against the 40th-percentile threshold and coach practice managers on working their gap lists.

MeasuresAIS-E: Adult Immunization Status
Q5LEAD, MSSP, Stars, and HEDIS all want the same clinical measures. Where do we get credit four times?

Controlling Blood Pressure and diabetes glycemic control are LEAD eCQMs, MSSP APP Plus measures, Star measures, and HEDIS measures. Breast and colorectal screening and depression screening are in APP Plus and Stars (Depression Screening and Follow-Up joins Stars for the 2029 ratings). CAHPS is in LEAD, MSSP, and Stars. Readmissions and follow-up after exacerbation are LEAD claims measures and the drivers of the 3x-weighted HOS results plans are graded on. NCQA is retiring chart-sample reporting for blood pressure in MY2028 and glycemic status in MY2029, so structured capture is the common requirement.

How Nelangie Cares closes the gap

We build one measure crosswalk for your network, one documentation standard, and one monthly gap list that serves every contract. Practices see one program; you report to four.

MeasuresCBP: Controlling High Blood PressureGSD: Glycemic Status AssessmentBCS-E: Breast Cancer ScreeningCOL-E: Colorectal Cancer ScreeningDSF-E: Depression Screening & Follow-UpTFU: Timely Follow-Up after Acute Exacerbation
Q6LEAD caps risk score growth and MA plans lost chart-review diagnoses. What does that mean for documentation?

LEAD uses a prospective model derived from CMS-HCC V28 with a symmetric 3% cap on risk score growth (4% for High Needs beneficiaries), and CMS will phase in AI-inferred risk adjustment from 2028 to 2031. MA plans can no longer count unlinked chart-review diagnoses in 2027, and every contract gets a RADV audit. Both programs now reward the same thing: accurate, encounter-based documentation of conditions that are actually present, captured in structured data at a real visit.

How Nelangie Cares closes the gap

We build one documentation standard that satisfies LEAD, MSSP, and MA: prospective suspect-condition lists, V28-aligned templates, MEAT documentation, and a quarterly coding integrity audit. The cap makes accuracy, not volume, the goal, and the same visit produces the structured vitals and results your quality measures need.

Q7Our MA plan partners are rewriting quality gates around Stars. What changed in what they will ask of us?

With eleven administrative measures leaving, plans have fewer levers they control from a call center and more that sit in your practices: blood pressure and glycemic control, kidney evaluation, screenings, three risk-adjusted adherence measures, functional status, polypharmacy, and the 3x-weighted HOS measures. Cut points on HEDIS measures rose in both the 2026 and draft 2027 ratings. Expect 2027 contracts to raise thresholds, add adherence and HOS-driver gates, and tie more of the deal to hitting them. Plans that fall below 4 stars lose the 5% bonus and drop from a 65% to a 50% rebate share, and they renegotiate downward.

How Nelangie Cares closes the gap

We build a network Star measure playbook with practice-level targets set against the new cut points and a monthly gap report by practice and measure, and we negotiate from documented performance rather than accept plan thresholds as given.

MeasuresCBP: Controlling High Blood PressureGSD: Glycemic Status AssessmentKED: Kidney Health EvaluationADH-3: Medication Adherence (diabetes / RASA / statins)COA: Care for Older Adults
Q8Improving is not enough anymore. How do we outpace the market across 40 practices?

Stars is graded on a curve, Tukey deletion keeps removing the lowest contracts before cut points are set, and roughly 10% of 2026 enrollees were in products that were discontinued, which mechanically raises the average. In LEAD, a fixed ten-year benchmark and first-dollar losses raise the cost of a practice that stands still. The practices that move the network are usually the bottom third, and their problems are workflow problems: unseen patients, unstructured data, no follow-up after discharge, and no owner for the gap list.

How Nelangie Cares closes the gap

We rank your practices by contribution to each measure, then deploy practice transformation sprints in the ones with the most headroom. Inside each practice we diagnose from its own data, align the team, redesign and pilot the workflow, and stay until it holds. ACOs engage us as fractional regional quality leadership or for defined sprints.

Q9The HOS measures are now 3x for our plan partners and LEAD measures days at home. How does an ACO influence outcomes it never sees directly?

Improving or Maintaining Physical Health and Mental Health compare a member's own answers two years apart, case-mix adjusted; LEAD's days at home and unplanned admission measures are claims-based. The drivers are the same: whether functional decline, pain, falls, and depression were identified and managed, and whether the patient stayed out of the hospital and the SNF.

How Nelangie Cares closes the gap

We make functional status assessment, fall risk, pain, and PHQ screening standard rooming steps at every Medicare visit with a documented plan for each positive finding, and we target chronic care and transitional care at the multi-condition patients who drive both the survey results and the claims measures. The new Star Functional Status Assessment measure and the 2029 Depression Screening measure are covered by the same steps.

MeasuresCOA: Care for Older AdultsDSF-E: Depression Screening & Follow-Up
Q10Adherence measures are being risk-adjusted and reweighted. How should we plan the next three years?

For the 2028 Star Ratings (MY2026) the three adherence measures are risk-adjusted for sociodemographic factors, lose the inpatient/SNF adjustment, and drop to 1x weight for one year; in the 2029 ratings they return to 3x. Plans will treat MY2026 as the reset year. Networks with large dual-eligible and low-income panels benefit from the adjustment only if their actual fill rates keep pace, and the same medication management supports LEAD's cardiovascular Prevention and Quality Plan track.

How Nelangie Cares closes the gap

We build adherence into chronic care management as a standing task: medication reconciliation at every touch, 90-day supplies and pharmacy synchronization requested at the visit, refill gaps worked from a monthly list, and outreach coordinated with plan pharmacy teams. We track proportion of days covered by practice so the network sees adherence moving before the plan reports it.

MeasuresADH-3: Medication Adherence (diabetes / RASA / statins)
Q11The Health Equity Index reward is cancelled and LEAD dropped the equity benchmark adjustment. Do we keep funding equity work?

Only the work that pays through the measures. CMS estimates the Star changes add about $18.6 billion in bonus payments over ten years and projects roughly a quarter of contracts down a half star. The cancelled reward and the absent LEAD adjustment remove bonuses for serving dual, low-income, and disabled members well, but risk-adjusted adherence, blood pressure and glycemic control in those panels, and survey drivers in multilingual communities still decide the rating and the withhold. LEAD's High Needs designation, with its own benchmark and 4% cap, is where that population's cost and quality now sit.

How Nelangie Cares closes the gap

We redirect equity work toward the measures that still pay: multilingual, AI-supported outreach under nurse oversight, LOINC-coded social need screening (the NCQA standard from MY2027), and a High Needs care model with home and telehealth touches, advance care planning, and transitional care after every admission.

Questions from payors

For an MA organization, the 2027 Star changes move the rating out of the call center and into the network. The measures that remain, and the cut points that keep rising, are decided in contracted providers' exam rooms.

Q1Our draft 2027 preview shows the same scores and a lower rating. What happened, and what will happen again in 2028?

Half of the draft 2027 cut points tightened; Kidney Health Evaluation and the respecified Colorectal Cancer Screening moved 7 to 11 points because neither is guardrail-protected as a new measure. In 2026, 57% to 60% of cut points rose. Tukey deletion removes the lowest contracts before clustering, plan exits removed about 10% of enrollees from the pool, and CMS is exploring a percentile-based method from MY2027. The bar will move again next year. A rating strategy built on maintaining performance is a strategy for losing a half star.

How Nelangie Cares closes the gap

We set network targets against a forward projection of cut points rather than last year's thresholds, and we prioritize the measures with the least guardrail protection and the largest weight. Then we do the part plans cannot do from headquarters: rebuild the workflow in the practices that determine those rates.

MeasuresKED: Kidney Health EvaluationCOL-E: Colorectal Cancer Screening
Q2Eleven measures are leaving. Which of the remaining ones can we still move ourselves, and which now depend on providers?

What you can move from the plan: Part D adherence outreach (though the measures are risk-adjusted from MY2026 and your practices influence fills), CAHPS access and coordination items, and the operations behind the survey experience. What now depends on providers: blood pressure and glycemic control, kidney evaluation, eye exam (chart review no longer counts), breast and colorectal screening, functional status assessment, polypharmacy and opioid-benzodiazepine deprescribing, depression screening from MY2027, and the 3x-weighted HOS measures, which are driven by whether chronic conditions and functional decline were managed in primary care. With the administrative measures gone, the provider-dependent share of your rating is the majority.

How Nelangie Cares closes the gap

We run plan-funded provider enablement in your network: gap reports practices can actually work, EHR templates that capture the measure in structured fields, staff training, and practice-level scorecards that let your Star team direct incentives to the practices with the most headroom. We are clinicians inside the practice, which is why the gap lists get worked.

Q3We lost chart-review diagnoses and NCQA is retiring chart-sample HEDIS reporting. How do we protect both risk and quality data?

Unlinked chart-review diagnoses no longer count toward risk scores in 2027, and NCQA is retiring hybrid reporting: statin measures in MY2026, blood pressure in MY2028, glycemic status and Care for Older Adults in MY2029, fully digital by MY2030. Both changes reward the same thing: structured, coded data captured at a real encounter and transmitted electronically. Supplemental data feeds from practices become your primary HEDIS source, and their quality becomes your rating.

We audit data capture in contracted practices and fix templates so vitals, results, screenings, and social needs are discrete and coded, then confirm the supplemental data path to the plan. The same visit-based documentation standard that supports a RADV-proof diagnosis also supports an ECDS-reportable blood pressure. One workflow, two revenue streams protected.

Q4The Health Equity Index reward is cancelled. Do we keep funding equity programs?

Only the ones that pay through the measures. The reward that would have replaced the reward factor is gone, but the risk-adjusted adherence measures, the survey measures in multilingual communities, and blood pressure and glycemic control in dual and low-income panels still decide the rating. CMS also removed the equity analysis requirement for utilization management committees. The money now follows outcomes in those populations, not the program label.

How Nelangie Cares closes the gap

We refocus equity investment on the practices serving your dual, low-income, and non-English-speaking members, with multilingual AI-supported outreach under nurse oversight, LOINC-coded social need screening, and care management that closes the clinical gaps those members carry. The same work improves the risk-adjusted adherence and HOS results that replaced the reward.

Q5HOS is now 3x and we only see it every two years. How do we plan for a survey with a lag?

The 2027 ratings use a cohort surveyed in 2024 and 2026; the 2028 ratings use a cohort surveyed in 2025 and 2027. What your network does in the second half of 2026 and in 2027 shows up in ratings two years out. The measures reward members who stay at the same physical and mental health level or improve, adjusted for case mix, which means preventing functional decline, managing pain and depression, preventing falls, and keeping members out of the hospital.

How Nelangie Cares closes the gap

We treat HOS as a two-year care management measure. In contracted practices we make functional status, falls, pain, and depression screening standard at every Medicare visit with a documented plan, and we run chronic care and transitional care programs for the multi-condition members most likely to decline. The new Functional Status Assessment Star measure reports on the same step, so the plan gets a leading indicator while it waits for the survey.

Q6What does a half-star swing cost us, and how do we build a case for investing in providers instead of another vendor?

A drop below 4 stars removes the 5% quality bonus and cuts the rebate share from 65% to 50%; a drop below 3.5 cuts rebates further. The average bonus is $381 per enrollee in individual plans and $466 in employer plans, and only 68% of enrollees are in bonus-status plans in 2026, the lowest since 2018. Analysts describe a half-star swing at a large insurer as hundreds of millions of dollars a year. Every remaining measure with rising cut points sits in the practice, which is where the next dollar of Star investment returns the most.

How Nelangie Cares closes the gap

We quantify your network's contribution to each measure and model what moving the bottom third of practices to the network median does to the rating and the bonus. We deliver the practice-level program, report monthly by practice and measure, and structure the engagement so the investment is tied to the practices and measures that move the rating. Not a vendor portal, a clinical team in the practices your rating depends on.

Q7CAHPS still counts at 2x and two items are leaving. What should we stop and start?

Customer Service and Rating of Health Care Quality leave for the 2029 ratings; Getting Needed Care, Getting Appointments and Care Quickly, Care Coordination, Rating of Health Plan, and Rating of Drug Plan remain at 2x. The survey runs web, mail, then phone, and case-mix was re-estimated for 2027. The items that remain are about access and coordination, which are practice behaviors: timely appointments, results explained, specialists' information in the chart, follow-up after a visit.

How Nelangie Cares closes the gap

We build access and coordination into practice operations: same-week visits for chronic members, closed-loop referral tracking, after-visit summaries in the member's language, and post-visit follow-up calls as part of care management. Those are the same touches that move HOS and clinical gaps, so the plan pays for one program and earns on three measure groups.

How Nelangie Cares closes the gaps

The six areas where plans must outpace the market are one workflow inside the practice, and that is where we work. Nelangie Cares is a physician-led practice transformation company. We connect your EHR, claims, and quality data, sit with your clinicians and staff, rebuild the workflow inside your own system, and measure the result in the metrics you are paid on.

Notes and sources

Figures are as of September 20, 2026. The 2027 Star Ratings cut points are draft (plan preview) and final ratings are expected in early October 2026; the percentile-based cut-point method is exploratory; no analyst source gives a confirmed per-member cost of a half-star drop, so bonus-per-enrollee figures are used as the proxy. This guide is educational and is not legal, actuarial, or coding advice. The reference infographic supplied by the client ("The New Star Ratings Reality") informed the framing of the six areas and the "improve faster than the market" thesis.
