About Nelangie Cares

It started with what we couldn’t ignore.

A healthcare consulting firm specializing in practice transformation, quality improvement, risk adjustment, and complex care operations — built for value-based care and Medicare Advantage performance.

Our story

Helping practices see what they couldn’t see alone

Providers stretched too thin. Workflows creating more confusion than care. Quality metrics, compliance requirements, and operational data sitting untouched — not because no one cared, but because no one had translated it into action.

So we asked a simple but powerful question: what if someone helped practices see what they couldn’t see alone?

We learned to read data the way clinicians read vital signs — with precision, urgency, and a deep understanding of what the numbers are quietly trying to say. We stepped into struggling programs and rebuilt them from the inside out: redesigned workflows, filled unnamed gaps, and built the operational structure that lets real care happen.

The results followed: fewer hospitalizations, better outcomes, and organizations recovering from financial deficit. And we are here to help you prove it.

Clinician-led

Built by clinicians. Run like operators.

Our work is clinician-led: trained in primary care and emergency medicine, then built through years of U.S. value-based care operations — care transitions for a high-risk population in Brooklyn, a 20-person care-coordination team in Florida, program design and compliance work for medical groups, a 26-practice public-health transformation portfolio in New York City, and quality and risk-adjustment performance for a multi-site provider network under ACO and MSSP contracts.

The premise is simple: the fix belongs inside the EHR and the visits you already have, not in a new platform, and it should pay for itself within the year.

  • Clinician-trained eyes on your data

We read data the way clinicians read vital signs — with precision, urgency, and a sense of what the numbers are quietly trying to say.

  • Hands-on, inside your workflows

We don’t consult from a distance. We sit with your teams, redesign workflows together, pilot them, and shadow them until they hold.

  • Every metric tied to money

Quality, risk, and utilization improvements are explicitly linked to bonus uplift, withhold recovery, revenue, and savings.

  • Built to outlast the engagement

The loop — data to redesign to reliability and back — is designed to keep improving after we step back.

How we work

Six steps in a loop, at whatever scale the client needs

  1. 1

    Listen

Your data and your day: what you are paid on, where the gaps sit, who does what at rooming.

  2. 2

    Fit

One plan tailored to your size, payer mix, EHR and staff. Not a package.

  3. 3

    Build inside

Workflows, templates and reports in your own system, with your own people, tested on real clinic days.

  4. 4

    Launch

Pilot first. First claims reviewed together before they go out.

  5. 5

    Prove it monthly

One working list, one scorecard, one standard that satisfies every payer at once.

  6. 6

    Stay until it holds

We remain through the pilot until your team runs it without us, then step back to oversight.

Multi-site provider network, New York

2025–2026 · Quality and risk-adjustment performance under ACO/MSSP

Situation

A network carrying total-cost-of-care risk under ACO/MSSP and value-based contracts, with quality and risk work spread across coding, data operations, compliance and revenue cycle.

What was done

Owned the quality and risk-adjustment workstreams through executive review. Provider-side counterpart to health-plan teams on EDPS submissions, reject resolution and risk reconciliation. Built Power BI dashboards, registries and KPI reporting from measure specifications, and Epic smart sets, order panels, note templates and registries carrying MEAT prompts, AWV scripting and ECDS workflows.

What changed

25% PMPM improvement · 25% MLR reduction · 15% fewer EDPS rejects · 12% fewer avoidable ADT events · +5% member incentive revenue

26-practice portfolio, New York City public-health program

2024–2025 · Practice transformation

Situation

Twenty-six independent practices at different starting points, and one program leadership team needing a single view of where every site stood.

What was done

Current-state assessments and practice interviews; clinical process redesign, EHR enhancements and hands-on staff training delivered on site; practice-level scorecards and leadership readouts with interventions sequenced by opportunity size.

What changed

10–15% quality-measure improvement · +15% operational efficiency · +5% revenue across the portfolio

30+ provider medical group

2024–2025 · CCM and RPM redesign, compliance and risk reconciliation

Situation

Existing CCM and RPM programs that were not paying for themselves, and open questions about whether submitted risk was supported by documentation.

What was done

Redesigned both program models with a financial model projecting per-program revenue, staffing and utilization impact; directed coding and billing compliance review; led risk-adjustment reconciliation with payer partners.

What changed

40% reduction in hospitalizations and readmissions · 15% revenue recovery

Care-management company

2023–2024 · Launching CCM, RPM and preventive programs from zero

Situation

No programs, no policies, no benchmarks.

What was done

End-to-end launch across clinical, billing and administrative workstreams; CMS-aligned policies and process documentation; KPI tracking and executive dashboards; revenue-cycle and clinical documentation improvement review.

What changed

+50% operational efficiency · 30% fewer readmissions · +30% program revenue

Care-coordination team, Florida

2022–2023 · CCM and RPM operations
