Independent and group primary care practices, whether paid fee-for-service, in MA capitation, or in an ACO.
PCP 1Will my Medicare Advantage revenue go up or down in 2027?
What is changing and how it affects your revenue
The headline is a 2.48% average increase in plan payments, but the number that reaches your practice is different. Plans lose the diagnoses they used to add through unlinked chart reviews (about 1.5% of revenue), V28 continues to pay less for many common conditions, and the big carriers are pricing 2027 bids for margin recovery. In practice that means capitation rates that grow slower than your costs, more quality withholds, and contract language that pushes RADV recoupments down to you. If you are paid fee-for-service, the proposed PFS conversion factor drops 1.19% to 1.68%.
How Nelangie Cares closes the gap
Nelangie Cares starts with a revenue exposure review: which of your MA contracts are percent-of-premium or risk-adjusted, how much of your risk score depended on plan chart reviews, and where your quality withholds sit. We then redesign the visit workflow so every diagnosis that is still payable under V28 is assessed, documented, and coded at the face-to-face visit, and we build the quality gap and pending-appointment reports your team works every month. The goal is that your 2027 revenue rests on what happens in your exam room, not on retrospective plan activity that no longer counts.
PCP 2What does the end of unlinked chart review diagnoses actually mean for my practice?
What is changing and how it affects your revenue
Beginning with 2027 payments, a diagnosis only counts if it is linked to a real encounter (yours, a specialist's, or a hospital's). A plan can no longer send a coder through last year's records and add conditions that were never addressed at a visit. If your patient with CKD stage 3 or heart failure was not seen and documented this year, that condition disappears from the risk score, and so does the payment tied to it. For capitated practices that is direct revenue; for fee-for-service practices it shows up as plans leaning harder on you for annual visits, suspect-condition outreach, and documentation quality.
How Nelangie Cares closes the gap
We build a prospective recapture workflow: a pre-visit suspect list drawn from your EHR, claims, and pharmacy data; point-of-care prompts inside your templates; and MEAT-standard documentation (Monitor, Evaluate, Assess, Treat) so every chronic condition is supported. We train your medical assistants and providers in a two-day onboarding, then run a coder-to-clinician feedback loop each month. We also set a visit cadence of four to six touches a year for chronic patients so nothing waits until December.
PCP 3I keep hearing about RADV audits. Am I exposed, or is that the plan's problem?
What is changing and how it affects your revenue
It is now both. CMS audits all roughly 550 MA contracts every year, samples up to 200 records per contract, and is clearing payment years 2020 through 2025 on a rolling schedule through 2027. Auditors request the medical record from your office, and an unsupported diagnosis is recouped from the plan. Most 2027 provider contracts include clauses that pass that recoupment, and sometimes penalties, to the practice. Extrapolation across the whole contract is on hold after a 2025 court ruling, but CMS has appealed, and even sample-only audits already reach your charts.
How Nelangie Cares closes the gap
Nelangie Cares runs a documentation integrity audit on your risk-adjusted population, scoring each HCC against RADV support standards and flagging the ones a reviewer would reject (unsigned notes, conditions listed without assessment, copy-forward problem lists). We fix the templates that create those gaps, train the team, and set up a record-retrieval process so an audit request is a same-week task instead of a fire drill. We also review the RADV and clawback language in your plan contracts before you sign.
PCP 4Which Star Ratings measures should I focus on now that CMS removed a dozen of them?
What is changing and how it affects your revenue
CMS removed administrative measures (call center, appeals, complaints, disenrollment, several CAHPS items), which means the clinical measures you control now carry more of the plan's rating and of your incentive dollars. The measures that matter for 2027 and 2028: Controlling Blood Pressure, HbA1c control, Kidney Health Evaluation, Eye Exam (kept after pushback), breast and colorectal cancer screening (colorectal is respecified), the three Part D adherence measures (statins, RAS antagonists, diabetes medications), and the new Functional Status Assessment, Polypharmacy, and Opioid-Benzodiazepine measures. Improving or Maintaining Physical and Mental Health, which come from the patient survey, now weigh 3x. Depression Screening and Follow-Up arrives for the 2029 ratings.
How Nelangie Cares closes the gap
We convert this list into a monthly working file for your practice: who is uncontrolled, who is due, who has not filled a statin, who is on more than one anticholinergic, and which appointments are pending. We redesign the rooming workflow so blood pressure recheck, PHQ screening, fall risk, and functional status are captured before the provider walks in, and we align your pharmacy outreach with plan adherence calendars. Because the physical and mental health measures are survey-based, we also coach the team on the care experience that drives those responses.
PCP 5The fee schedule is going down again. How do I protect fee-for-service revenue in 2027?
What is changing and how it affects your revenue
Under the proposed rule the conversion factor falls 1.19% for clinicians in an advanced APM and 1.68% for everyone else. G2211, the visit-complexity add-on, becomes a modifier worth 16% of the base E/M, or 32% if you participate in MSSP or LEAD, which is a meaningful reward for ACO participation. New codes cover advance care planning by clinical staff and shared medical appointments. Care management codes (CCM, PCM, TCM, APCM) were exempt from the 2026 efficiency cut and remain the most durable primary care revenue line.
How Nelangie Cares closes the gap
Nelangie Cares builds the care management program most practices leave on the table: Chronic Care Management with prefilled, condition-specific care plans (hypertension, diabetes, heart failure, atrial fibrillation, COPD, hyperlipidemia), Transitional Care Management after every discharge, Annual Wellness Visits with integrated social-needs screening, and Advance Care Planning. We handle enrollment scripts, consent, time tracking, and the billing rules (for example, CCM and TCM cannot both be billed in the same month). For multilingual panels we deploy AI-supported language tools so staffing cost does not eat the margin.
PCP 6Should I join an ACO in 2027, and which one?
What is changing and how it affects your revenue
For most independent primary care practices the answer in 2027 is yes. MSSP participants would get the 32% G2211 modifier, are exempt from the larger fee cut, and can now offer patients Part B cost-sharing support from April 2027 under the proposed rule. The trade-off is that one-sided (no downside) risk in the BASIC track is now capped at five years for agreements starting in 2027, and CMS is ending prepaid shared savings after the 2027 cohort. LEAD, the 10-year successor to ACO REACH, closed applications in May 2026, so joining it for 2027 means becoming a participant TIN in an ACO that was accepted; participation is whole-TIN, a TIN cannot be in both MSSP and LEAD, and Professional-option LEAD ACOs pay primary care through capitation rather than fee-for-service. CMS expects future LEAD application windows but has not announced dates.
How Nelangie Cares closes the gap
We run an ACO readiness assessment: your attributed lives, cost benchmarks, quality reporting capability, and which track or ACO partner fits. We check your assignment position (which of your codes count as primary care for attribution, including the new 2027 additions), compare the MSSP and LEAD participation terms on offer in your market, and prepare your practice for capitation cash flow if a LEAD ACO is the right fit. Once you are in, we build the eCQM and Medicare CQM reporting workflow and the monthly performance review so shared savings become predictable instead of a surprise at reconciliation.
PCP 7What is changing with prior authorization, and does the WISeR model affect me?
What is changing and how it affects your revenue
For MA, the CY 2027 rule did not add new prior authorization rules, but the CMS-0057 interoperability rule still requires plans to run an electronic prior authorization API by Jan 1, 2027, with 72-hour urgent and 7-day standard decisions and specific denial reasons. WISeR, the AI-assisted prior authorization model in traditional Medicare, is live in Arizona, New Jersey, Ohio, Oklahoma, Texas, and Washington for services such as skin substitutes, epidural injections, and nerve stimulators, though Congress is trying to defund it. The mandatory Ambulatory Specialty Model that starts in 2027 also nudges cardiology and pain specialists toward electronic prior authorization.
How Nelangie Cares closes the gap
We map your current authorization workflow, identify the services and payors that generate denials, and rebuild the intake so requests go out complete the first time. We create a denial log with root-cause categories and review it with you monthly. For practices in WISeR states, we set up a referral pathway that anticipates the documentation the vendor will ask for, so patient care is not delayed.
PCP 8Plans are exiting my market and patients are switching. How do I keep my panel stable?
What is changing and how it affects your revenue
Humana, UnitedHealthcare, Centene, and Aetna are all trimming plans, counties, and benefits for 2027. During the Oct 15 to Dec 7 enrollment period, patients will change plans, some will land in plans where you are out of network, and the ones who switch MA organizations fall under a special risk adjustment exception in 2027. Every switch resets your quality denominators, attribution, and, for capitated practices, revenue.
How Nelangie Cares closes the gap
We build a panel management routine: a roster reconciliation each month, outreach to patients whose plan is terminating, and a January-through-March visit push so new and switching patients are seen early, their conditions documented, and their care gaps opened for the year. We also make sure your practice is listed accurately in plan directories, which became a bigger driver of enrollment as plans changed their benefits.
